Release of Liability

• You and/or your child will be participating in a professional candle-making environment, utilizing professional equipment such as wax melters. The use of this equipment will be monitored at all times, and proper safety methods will be taught consistently. Be aware that the fragrance oils used in our candles can cause allergic reactions or respiratory issues in sensitive individuals. Additionally, the equipment and materials used, including hot wax, are hazardous and can cause burns or other injuries if mishandled.

• ReWax and UnWine reserves the right to use photographs and videos taken during classes, workshops, and other events for publicity materials and social media purposes. Registration in classes constitutes consent to the use of these images without further permission or compensation.

• Participation in candle-making events, workshops, candle-making kits, and other activities is at the sole risk and liability of the individual and/or the parent or legal guardian enrolling the child. ReWax and UnWine, its affiliates, franchise network, franchisor, and employees accept no liability for any injury, illness, or damages you or your child may suffer related to the premises of the event or participation in activities sponsored by ReWax and UnWine. This includes, but is not limited to, slip and fall incidents on the premises, burns from hot equipment, and allergic reactions from fragrance oils.

• Due to the potentially hazardous nature of hot wax melters, dyes, hot wax, and fragrance oils, it is the responsibility of the registering individual and/or parent or guardian to ensure the appropriate placement of the individual and/or child based on their skill level. Every effort will be made by ReWax and UnWine and its employees to ensure the safety of you and/or your child while on the premises. However, we are not responsible for any damages to personal property, including spills on clothing, that may occur during participation.

 

Candle Care Acknowledgment

• To ensure safety and proper use, please adhere to the following guidelines:

• Burn candles for no more than 4 hours at a time and keep away from flammable materials and drapes.

• To prevent smoking and soot buildup, keep the wick centered and trimmed to 1/4 inch.

• Keep candles out of drafts and away from children and pets.

• Do not move candles while burning or if the wax is hot.

• Discontinue use when only 1/4 inch of wax remains.

• Never leave a burning candle unattended.

• ReWax and UnWine, its affiliates, franchise network, and franchisor are not responsible for any house fires, building fires, or other incidents that may occur after you leave with your candle. Any damages or injuries resulting from the use of the candle after leaving our premises are solely your responsibility. If the candle breaks after you leave, we are not obligated to replace it.

 

Waiver and Release

• I hereby waive, release, and discharge any and all rights and claims against ReWax and UnWine, its affiliates, franchise network, franchisor, and employees for any damages, injuries, death, or permanent disability which may occur as a result of participation in activities at ReWax and UnWine. This waiver is intended to release ReWax and UnWine, its affiliates, and franchisor in advance from any and all liability arising out of or connected to the activities at ReWax and UnWine.

• I understand that participating in candle-making activities involves certain risks and hazards which may result not only from my own actions, inactions, or negligence but also from the actions, inactions, or negligence of others, or the condition of the premises or any equipment used. Furthermore, there may be other risks not known to us or not reasonably foreseeable at this time. ReWax and UnWine, its affiliates, and franchisor are not responsible for any accidents or incidents, including those involving overconsumption of alcohol, burns, spills, or slip and falls. Participants must be 21 years of age or older to consume alcohol. If a participant is under the influence of drugs or alcohol and drives home, resulting in injury or damages, ReWax and UnWine is not responsible.

• I acknowledge that I have read and understand the above provisions in this waiver and consent to liability, and agree to abide by them and sign of my own free will.

• I knowingly, voluntarily, and expressly waive any claim I may have against ReWax and UnWine, its affiliates, owners, franchise network, franchisor, and class/workshop sponsors for any injury or damages that I may sustain as a result of participating in the program. I also acknowledge that I have read and understood the candle care instructions.

• By signing this document, I affirm that I have read and fully understand its contents, and I agree to the terms and conditions herein.

Lastly, Maryland State law requires explicit consent to audio record people.

Consent Details: I, the undersigned participant, hereby consent to the audio recording of my voice in the designated areas for the purposes of security surveillance.

Rights and Permissions: I understand that my recorded voice may be used for the following purposes:

• Security monitoring and management

• Incident investigation

• Legal and regulatory compliance

Confidentiality and Use of Recordings:

• I understand that recorded audio may be stored securely and accessed only by authorized personnel.

• Recordings may be shared with law enforcement authorities or legal representatives as required by law or for security purposes.

Duration of Consent: This consent shall remain valid indefinitely unless revoked by studio owner in writing.
